If you're talking about the insta-kill throat slit, you might not have the needed skills leveled high enough or be putting out enough damage. Helps to at least have the Backstab perk from the Sneak tree.
I have pulled off the throat slit before, i just dont know how to pull it off again, or another sneak attack. Do you use stamina to do it? or is it automatic once your behind your target?
I have about 12 companions following me, and they get detected frequently but I'm still able to get the sneak attacks since I guess I just sneak well enough.
If the NPC you're attacking is too high a level and you're damage isn't high enough with your dagger / sword, etc, it won't give you the throat slit. You still get a bonus for damage, but not the insta-kill. At least that's how it's worked in my game.

Ahh i get it now :) so my targets are all too high for me to do throat slit. well dam -.- It was helpful thank you Stabbykitteh :) i just need to find better daggers/one hand swords to do so. i just have trouble finding good daggers/swords. Know where to find some? im lvl 14 and doing companion quests for now.
Your best bet for good weapons is to make them yourself. Dont rely on finding them in chests or at the shops. Pile up the componets needed to make them at the forges and run up your smithing. Always, ALWAYS improve your gear whenever you can to keep it the best possible. Smithing is usually one of my first 3 skills to hit 100 every time. (Speech, Smithing and Enchanting) these skills are super vital reguardless of what type of character you're playing if you want the best possible gear at all times and the best deals on buying and selling (Speech).
Tip for running up your speech skill faster:: Always sell items (esp when the skill is low level) ONE AT A TIME. not in groups. you get skill progress for each barter you do and while the more pricey the item the better the bump in skill, it's still better to sell lots at a little than one pile for alot. ALSO try to persuade or intimidate every chance you get... these bump up the speech dramatically when successful.
High Speech. Smithing and Enchanting will make your coin purse fat in a hurry too and you wont be worrying about having the coin needed for training sessions, house buying or anything.
