Dunno about the coughing. I don't believe that NPCs can contract deseases the way the player can, and I don't think even the player can spread deseases. I don't know for sure, but I think deseases work as a race linked dice-roll encounter mechanic rather than as an actual contagion. That is, for example, you can only get rockjoint from wolves, and the game doesn't actually assign specific wolves to be rockjoint carriers, it just rolls a dice every time you fight a wolf to see if you contract rockjoint from the encounter. if wha you're fighting isn't a wolf, then it doesn't roll for rockjoint.
From my own experiences with deseases in game, I don't think any of them (except vampirism) change textures/shaders or animations to "display" the sickness. You just get stat modifiers and the ocassional "you don't look well" comment from NPCs. I suspect there some kind of randomizer determining what idle animations are played for an NPC, and for some reason Lydias is getting stuck. My Lydia is a yawner like Master Dan's, and I had pretty much the exact same response to it as he did, LOL.
As for stopping it, I dunno. Try dismising her, cleansaving, reloading, then rehiring her. If that doesn't work, and the coughing really bugs you, leave Lydia at home and hire Janessa (the psycho dark elf mercinary in The Drunken Huntsman). She's pretty badass, and has a bit more personality than Lydia.
The coughing is just a little "human" thing to make NPCs seem more real. She's not sick, she won't die from it and you can't catch it from her. Personally, I don't tend to travel with Lydia, she annoys me senseless, but I have had other followers do it occasionally.
