Dragonbone Sword?
My Dragonbone Sword only upgrades to "epic" level. I have 100 smithing and what not and my Dragonarmor is at legendary level. Why is the sword only upgrading to epic instead of legendary where it should be?

You'll need the Glass perk to further enhance your weapons to that level.

Do you have a source for that? I'm at level 35 and 3 perk points are relatively hard to get right now.

I have 100 smithing without the Glass perk and my dragon item improvments are Legendary.

With the 'light' armored smithing invested I got Dragonbone Swords up to par currently to 905-964 dmg right now. The many factors contibuting to dps and the amount of damage you can smith depends upon your alchemy components from the skill tree, including the temporary smithing potion that potentially makes your weapons and own armor fortified much stronger. Coupled this with the Seeker of Might active effect from one of the Black Books in Solsteim it'll reach even up to 1k dmg

Unless there's another perk that improved it that I'm forgetting, that's all I do and I get Legendary improvments.

It's vital to enchant one-handed upgrades to your equipment and accessories at all possible gear to go past your current damage too man. A Blacksmith Elixir's strength only fortifies up to 50% maximum later at in-game apothocary shops or finding it randomly in chest. Making a better one couped with Alchemy Perks including Alchemist, Physician and Benefactor the equip and gear can be smithed stronger. Mines go up 184% improvement. Have gear fortified within Created Potions are x% more stronger (Chest, Falmer Helmet, circlet, rings, necklaces) so it'll increase the potion's effectiveness when you make it.

Does teh weapon have enchant in it? If it does, do you have Arcane smithing so that you can upgrade magical / enchanted weapons and armor.

If it has an enchantment and you don't have that perk you can't upgrade it at all.

Yes but if it was upgraded to epic before and enchanted later. Therefore it cannot be upgraded past that point unless teh perk.
