Did you ran LOOT? also did you try verifying cache? and do you have the unoffical patches installed?

Run LOOT.

LOOT
https://loot.github.io/

To verify your cache:
- From Steam client Library view, right click the game
- Properties > Local Files Verify Integrity of Game Cache...

1 file will always fail to validate it is related to how Steam detects your graphic card and does not have any effect to your game.

I've had some problems with a hometheater 5.1 that was not working properly. Somehow, Skyrim detects is the sound is 2.0 (stereo) or 5.1 and configures itself to work that way. I've noticed the problem because when someon behind me spoke I didn't hear nothing.
After removing the hometheater and configuring the PC to output 2.0 only, the game worked properly.

That fixed my problems with my game, especially the standing right in front of a NPC and not being able to hear a word they're saying, but then turn to the side and they're good and loud. Changing to 2.0 fixed that for me.

I'd have to guess that the OP's problem is related to one of the sound mods being corrupted, or even more likely, installed in the wrong load order. Climates of Tamriel is very picky about sound. One of the mods for sounds (Sounds of Skyrim) never would work with CoT, so I had to get rid of it.

Definitely check your load order with LOOT. If that doesn't fix it, try disabling some mods that affect sounds. If that doesn't fix it, then try reverifying your game cache files and running LOOT again if you change or add anything to your mods.

EDIT - bad news people. and some intresting news - it appears to have fixed the problem but only when playing sound through my monitor ( do not have speakers ). through my monitor everything works great the only downside is even on high settings the volume is relatively quiet.

ALSO it does not appear to work when i using my gaming headphones. which are.
TURTLE BEACH PX22's
looking in the default settings they are as follows.

16 bit. 48000 Hz (DVD quality).
with thes options both as ticked active..
Allow applications to take exclusive control of this device
give exclusive mode applications priority..

ANY further thoughts?

thanks agian.

Set your pc's system sound settings to use 2.0 stereo, meaning NOT 5.1 or 7.1 or any surround settings. Skyrim doesn't do well with a lot of surround settings. If you don't like the new (hopefully much louder) sound, you can always switch back to the surround version if you prefer. Usually switching to two channel stereo fixes that.

Oh, and there's another setting worth looking at. If you right-click your sound icon in the system tray (bottom right of Windows 7, same for most of them) and choose Sounds. There's a settings tab called Communications. In there, set the system to do Nothing and see if that helps too.

Contra panel ?
Hardware and sound?
change system and sounds?

could not see it in there.
sorry if obvious only just got into modding and gaming on pc

thanks again for all the help <3

My headset has a virtual 5.1, sometimes I've had to turn it on or off specifcally because a game's audio wasn't balanced. I have also noticed that when switching between the two settings sometimes the audio bugs and I have to flip the switch to fix it.

Like if I change to 5.1 during a loading screen, that creates the bug and I would flip the switch to fix it.

Same place, different tab. Instead of picking Communications, pick the Playback tab and choose your audio device for the speakers. Look for one that says stereo or 2-speaker setup...I swear I've seen different ones. On my laptop it's just listed as just Speaker/HP, but I think every system is different depending on individual features available. Look for one that's NOT a Surround type or 5.1 or 7.1.
