If it is a mod (or maybe, because you have some installed), then you should post a list of the installed mods (in their load order), and the tools you use to manage the game and mods (Workshop and/or NMM or MO, Wrye Bash, TES5Edit, etc.)
Once you get used to the left/right mouse button switch (left button controls right hand), the combat seems fairly intuitive, at least to me. Basically, point crosshairs at enemey and click to swing weapon.
Step 1: Unsheath weapon by pressing R
Step 2: Aim crosshairs at opponent
Step 3: Click left mouse button to swing weapon
Step 4: Click right mouse button to block with left hand
Step 5: Sheath weapon by pressing R again
Note: Some Bethesda games have you automatically unsheath weapon with a left mouse click, but I can't remember off the top of my head if Skyrim is one. But in any case, a left mouse click should NEVER sheath a weapon. If it is, my guess is that your controls settings are messed up.
