make a backup
open console
now you type in
*savegame backup*
now you have a hard save that is named backup instead of a number.
then type in *sexchange* and then *sexchange* again
should remove stuck visual effects
dont include the * in the console
I almost wish I had the issue if this will fix it. I hope the OP reports success.

Just remember, the warranty on console commands is if you break your game, you get to fix
it.

then its stuck in the save and as since you made that backup save you have the fallback point to before the sexchange command was run.
when effects gets stuck like that they are extremely hard to remove and usually the only option is to load a save before the effect was applied.
Yes I have, it has fixed it for me before but not anymore. And the sexchange isnt dangerous command
