Huge frame drop when looking at npc
I get around 60-70fps outside and 70-90fps inside, but whenever I enter a town or a city the framerate drops to 2-3fps. This can't be normal does anyone know why this might happen?

These are mods that I have that edit NPC.

Not exactly addressing your problem but you should be aware that having a fps more than 60 makes the game go crazy so you'll have to turn on V-synch to limit the fps.

How do I turn on V-sync?

The ones with X's at the end are not loaded

Get the unoffical patchs
Get LOOT and run it to fix your load order.

Get alternate start live another life to shortcut getting into town, and start disabling mods, starting a new game, going into a city and testing it and see if you can narrow it down

I used BOSS to fix the load order, but why do I need patches? It didn't always do this, in fact yesterday it was working fine with these mods.

Yes, I can tell by looking at it, but BOSS is outdated by over a year. LOOT is the only current working one. There is more information about this in the link which is why I both told you to get LOOT and provided a link.

And you need the patches because they fix upwards of 15 thousand bugs and its a nightmare trying to troubleshoot all of those bugs on top of anything else that could be causing this, when there is a patch out there for them already

I'll try the patches and see if that fixes anything, but what does 'Get alternate start live another life' mean?

Its a mod. Get it so you can do testing easier as it lets you get into a city quicker.

If I run nexus mod manager after using LOOT will it reset the load order?

No. LOOT and BOSS interact with NMM and other tools in the same way

I installed the patch and ran LOOT and I get normal FPS in towns and villages now!

Make sure you have all of the patches for the dlcs as well :)

Pfft. On a technical level, Skyrim is a disgrace of a game. Bugs everywhere. There was even a bug introduced in the Dragonborn DLC which stops you from ever absorbing dragon souls if it appears in your game, which of course ruins a good majority of quests and a core mechanic. The unoffical dragonborn patch fixes it :)

Yup, I can vouch for that, had that very issue, thankfully kind people on this forum pointed out I needed *all* the patches for my DLC, I reloaded to before I killed the first dragon, and it worked :)
