A lot of the really popular singular custom followers, like sofia and Inigo are ignored by follower managers so it doesnt matter. Some of the smaller packs that include followers like interesting NPCs include patches for follower systems (although at the moment interesting npcs is incompatable with UFO and AFT and only has a patch for EFF)
Othertimes, it depends on how the follower is constructed.
Generally EFF is pretty safe to be anywhere in your load order. Instead of breaking followers, if it encounters an issue with them it just ignores them. LOOT should set the order for EFF perfectly fine, it always has for me.
AFT should generally be towards the bottom. However that mod is VERY fiddly with its load order, and I suggest you get it set or stable before you start a game with it. Just changing its load order position mid game can be enough to break all of your followers.
UFO I have never personally used, but people have told me that while it isnt as touchy as AFT, having it at the bottom is generally safer as well.
