Open console then enter showracemenu.

Using showracemenu will let you change everything about your character except race and sex without losing any experience progress or anything else. Also, do not select a different preset in showracemenu unless you want to start over with a new character.

Actually, one of the DLC's (Dawnguard, I think), adds this ability. It takes the form of a NPC who is found in the Sunken Flagon and who will change the appearance of your character's face. No need to risk accidentally messing up your character via console commands.

Do you know the name of the NPC ?

I forgot her name, but she's a High Elf in the Ragged Flagon, under Riften. She will change your appearance for 1000 gold. More lore friendly, and you get an achievement :)
