BOSS is entirely obsolete for Skyrim. Don't use it.
LOOT stands for Load Order Optimization Tool. You do need that.
Mod managers, like Nexus Mod Manager (NMM) or Mod Organizer (MO) makes installing and uninstalling mods easier and safer than manual installation. This is important, because mods shared as loose files will physically overwrite otherwise, if there are several versions of certain files - and there often is.
Consult the "Tools" chapter from our pinned topic for more information. I would actually recommend that you read the whole post through.

Do you need to put Loot inside Mod Manager?
Or do they just "recongize" each other?
Chapter 3 will show you how to install LOOT, MO and more. If you want you could follow the whole guide to build yourself a stable game.

Depends on which one you use.
- NMM/Workshop: LOOT will run outside of the manager.
- MO: LOOT will need to run through MO, or it will not detect mods from virtual Data folder.
It's generally not recommended to use the built in version of LOOT that MO has, it's very outdated as I understand it, and seems not to sort well.
