Report a translation problem
If your horse does not spawn next to you or somewhere close by when you fast travel then that means it either died or it glitched/bugged somewhere.
Try Fast travelling somewhere else or loading a previous save file to see if your horse will spawn next to you after you fast travel.
If your horse does not appear next to you after you loaded the latest auto-save file then its probably dead. Try loading a more previous save file where you know its alive.
The game is many years old and it does tend to have a lot of bugs and glitches.
The fact the horse leaves the area or when you exit a cave to find your mount where you left it and its not there tends to happen a lot even to me and many people i know who play the game, its practically a common thing.
Now i dont know if there is a mod that can fix this but i have seen that there is a mod where you can "Whistle" to call your horse to come to you.

It happens every single time, no matter how peaceful an area.
I have fast traveled a lot, it will either leave or not show up at all, it's very annoying and inconvenient
type setownership and the horse will be yours.
Dead horses owned by the player will respawn at the corresponding stables that sold them, even though a new horse will have been generated (and/or sold) to replace them. The dead horses (now alive), have the original Ref. Nos. and the correct name ([Player]'s Horse), but they behave as if the horses are free horses (legal to ride but will return to original position once dismounted).
is fixed by it..
#2 does this problem happen in a NEW game?
Haven't tried making a new game, would rather not, planning to uninstall and install again, see if that fixes it
trying a new game, and buy a horse there (money you can give you with the console) will tell us if there is an underlying problem with the game or worse with your save game
Shadowmere didn't have the problem for a while, but then suddenly it started not spawning with me and then I just couldn't find it anywhere
if something acting up in the savegame, a file validation / reinstall will not fix it. only way in this case would be to find a save before the game was acting up. the worst possible case is to start a new character.

A good bit of my travel now is done by carraige to the nearest city to my destination and then on foot to where I am going. This gives me more opportunities to grab ingredients along the way too as well as take down some game for dinner. This travel method is fairly immursive too so I have gotten out of the fast travel habit with it.
