That's because of a mod you're using.
OP, you'll have to post your mod load order so people can help figure out what's wrong.
I just completely decided not to pick any dual cast perks as a solution.
BUT there is a workaround...
Reset one of your mouse buttons to a keyboard key (let's say "b").
Now, if you hit "b" and the other mouse button at the same time, you can dual cast. You still need to time it right, however, but it did work for me.

Unfortunately, I don't know. I play on a pc with keyboard and mouse.
It's worth switching around the controls to see if it works though.
1) Open you magic menu and go to the spell you wish to dual cast.
2) While hovering over the spell with your mouse, press the left and right mouse buttons at the same time. (You should see a "LR" appear next to the spell)
3) When casting press both mouse buttons simultaneously to use both hands to cast a single fire ball. Press each mouse button down at different times to cast two separate fire balls at the same time.

I suggest they relax the activation check to 0.5 seconds if not a full second for us older players.
