It either has to be the most valuable or most powerful weapon or armor item of its type, unless you have a mod that makes NPCs ignore value and damage ratings.
So, if you want them to use an Ebony bow, give them one, but they can't have a more powerful smithed-up Imperial bow. If you want them to wear leather armor, you have to remove their steel armor from their inventory.

MAK07's Remodeled Armors breaks almost all vanilla armors down into individual components like pauldrons, arm guards, belts, etc, and lets you mix and match, like Dragonplate cuirass with Daedric Pauldrons, or Elven and Glass combos. Granted, they're impractical skimpy armors and only work on female followers.
Still, they do blend with full vanilla pieces mostly, so you can put someone in full-body vanilla Ebony armor and give them Daedric Pauldrons, Gauntlets, and Boots. Or put someone in full Blades armor and give them Dragonscale Pauldrons.
Showinventory after clicking on the follower's face will bring up a list of all items they are carrying
with their ID numbers. At the end of a row it will show "worn" for an equipped piece.
Unequipitem (listed ID) 1 ... should unequip that item

I will add that using all these to change lots of npcs can start to make skyrim less stable as Skyrim needs to remember all changes. So use with care.
