Your character will have some sort of illness/sickness,

You can cure yourself by :

  • Praying at a shrine.
  • Drinking a potion of cure disease.
  • Talking to a vigilant of Stendarr
  • Becoming a Werewolf I guess

Hawk feathers are useful.

No offence, but you look kind of sick...shouldn't you be in bed?

You're a sick man, you know that?

Look at your active effects...any illness you have will show up in red on the list. That's your first clue that carrying a couple of cure disease potions is a good idea. You might miss that little notification that you contracted a disease and the people telling you that you look ill means you should do something about it.
It could be your character hasn't slept in a while, because in skyrim sickness and tiredness look the same !? (!? is an old fashioned convention that basically means puzzling or vexing).

Or if it's the alchemy shops, then it's an idiot script that they repeat whenever they don't have anything better to say.

I have this problem too. I don't have any sickness/diseases and I sleep regularly but random citizens keep telling me I look sick

There is one NPC in Whiterun, the woman who runs the apothecary, who has questions like that as part of her standard dialogue. Even in you're not sick, she'll often say that to you. It could be nothing, but I would visit a shrine or use a Cure Disease potion, just in case.

Arcadia makes that comment about looking a litlle pale as part of her regular dialog of being a healer/potion maker. "You look a little pale...could be ataxia....its quite common back in Cyrodil'" (or pretty close to that) Its just one of her chat up lines toward getting you to buy potions or ingredients.

Yeah, but it is random citizens on the street telling me this, I have visited two shrines and they still say it
