It just drains their stamina/magicka/health, reducing it, like a magical equivalent of a poison.
I don't use the Mace of Molag Baal, but that's because I can enchant a better weapon by the time I get to Markath. In fact most Daedric weapons are underpowered IMHO.

using this mod you can click on someone and get their stats. hit someone wait for the drain and click them to see they stat changes. this is how I make sure the effects are actuaaly working.
