If you select a one-off choice, one you can't undo, and didn't want, then reload.
Most importantly, dialogue scroll is now holding in place, which makes it less likely for game to use a wrong line instead of what you selected.

No. Mod(s) that I linked did fix it for me.
I have a long series of updates long overdue. Everything in that list is up to date, but I will be adding some content there soon.
If you are swapping ever, try to use one method. If that's not your issue, them I did my best.
It's usually your mouse's position when you start conversating. If it's too close to the conversation box then it can highlight an option on its own while the marker on the left remains on the original option. What I do is try to keep the mouse away from whatever window I'm looking at (inventory, magic, dialogue, etc), and if I think I accidently highlighted the wrong line I just scroll all the way to the bottom and back to the top (or in the case of the inventory/magic menus, side-to-side) to get the marker to line back up correctly.
