If the name is still there, then the planet is still there, when you change an average-mass planet into a black-hole, it becomes a speck in metaphorical terms. try just doing it manually, amp up the planet density a few hundred times, add ten or twelve zeroes to the mass, and keep the diameter the same as oh say earth, tell me if this helps.

Yeah, I tested this with an earth-mass planet, it didn't delete the planet. it shrunk it down to 1.5 cm diameter, condensing one earth-mass to black-hole density.

Place a star near a solar system and then turn that into a black hole.

A black hole has the same mass and gravity as the star which collapsed to make it. So if by magic our sun became a blackhole the light and heat would vanish but the motion/ paths of the planets around the sun would stay the same. It's the black holes that are wondering around that cause mayhem.
Want to make things even more interesting? Turn the Milky Way into a galaxy of black holes, and send it on a gauntlet run of every other galaxy in the universe, see who wins. :)
