For the first promotion, you need a total of 7+ of the Aspect of your cult.
So, for example, you're a Moth cult and you're trying to improve a cultist that already has Moth Aspect, you'd just need a Moth Lore with 6+ aspect.
You may use Trappings to help you achieve the necessary numbers, like objects or Influences.
Can you only promote people who have your cult trait to the highest rank?
Is there a limit on how many people of highest rank you can have?
You can only promote followers of your cult's aspect to Exalted (10 Aspect).
The process is done independently for each follower, so the only limit on how many Disciples/Exalts you can have is how many followers you have.
Oh, and promoting Pawns is risky and normally not worth it.
