Underlined means narrative event, which are scripted one-shots, not from RNG.
- At start of game, unlock Work, your sole card is Another Shift at the Hospital
- Work: Another Shift at the Hospital adds Recall my Dreams
- Recall my Dreams unlocks Dream, adds A Change in the Air
- A Change in the Air expiration adds A Bequest Arrives
- A Bequest Arrives expiration unlocks Study, adds A Bequest
- Study: A Bequest adds Notes on a Possible Collaborator +3 others
- Study: Notes on a Possible Collaborator adds An Acquaintance
- An Acquaintance expiration unlocks Talk, adds your 1st Acquaintance
So Talk is gated behind (a sequence of) all-narrative events. If you're in any game mode that omits any narrative from that chain, I guess it breaks.
I suppose Physician mode simply doesn't give you A Bequest.
What do you get instead of that?
Conjecture 1: Every Legacy has its own narrative script
Then a dev must have meant for Physician to win without Talk.
Conjecture 2: Other legacies omit a different verb each?
