97% journal entry

Not sure it has anything to so with Leviathan, I've completed it and I'm stuck at 97% as well.

I'm also stuck at 97% and think I have completed everything except for the Leviathans Landing.
I assume you already activcated all of the trophies (Book, Orb, Spear, Trident, Sword, Wicked K's hat) in the void to gain their journal entries?

yep. already interact with every posible object

Hmmm, interesting. You get an entry for each mission. You get an entry for each boss (following interaction). You *start* with entries on the Horsemen and some other entities. You do not seem to get anything for beating everything on Hard or Apocalyptic, or for finishing the Arena or the entirety of the Void itself (my experience).

What...I wonder...could possibly be missing?

I just had, and solved, the same issue.
At first glance in my journal I thought I had every entry (16 'Chapter' entries, 6 'The Void' entries and 4 'History' entries) but upon closer inspection while counting the number of 'Chapter' entries I saw that there were in fact TWO entries labelled "chapter 5" ("Reflection" and "Houndmaster") so even though I had 16 entries, I must be missing a chapter, and in my list I was missing chapter 14. So I went into chapter 14 and realised I had missed a conversation there. (It seems I initially missed it because it appears after you open a door, but it's located just before the door).
TLDR; There are in fact 17 'Chapter' journal entries in the journal because there are two for chapter 5. If you're stuck at 97% you probably missed one of the "War & Strife" discussions in the levels.
