16/17 levels beat on hard mode, 17/17 on mayhem

I just beat the game at Mayhem mode but I still don't get the achievement for the hard mode. I might have played one of the levels in the first run at a lower difficulty setting but I don't remember which one, as the game doesn't display at which difficulty I beat each level anymore.
It gave me the Mayhem achievement and every level has the mayhem symbol.

Is there a solution without having to play it all again at hard mode until I find the right level?

I'm also reading that in the last patch:
"Beating levels on Mayhem difficulty now correctly complete quests tied to Hard difficulty."

Is this a bug?

guess it is a bug since I did all levels in hard and it stills state 16/17 in quest part (achievement didn't unlock either)

I'm reading here on steam that the 17th level (boss) is in Mayhem mode only. So I don't get why the achievement asks for 17 missions on hard if they're only 16.

Shouldn't be. Worked fine the first four times I played through the entire game. Twice, on release, it counted as Hard and Apocalyptic. Twice, post-Mayhem-introducing-patch, it counted as Hard and Apocalyptic again. I went through every level in sequence and each time I completed one on Apocalyptic, it would count for Hard as well.

So unless it is only the most recent patch which reminds the game that Mayhem also counts for Hard that has broken this, making the game somehow forget that it counts for the secret boss...

Same here, 17/17 apocalyptic, only 13/17 hard
