I find this a little bit strange since sometimes I am sensitive to same games which indeed give me headaches, but this is not one of them.
Just to validate there weren't any changes to the game since I last played and I wasn't going crazy, I did open up the game again and yes, it's still the same problem.
So the slight stutters might be hard to detect, but the general scrolling/motion of the screen is so poor it should also come up on 60hz monitors as well. To try and detect this I looked up some gameplay of Kingdom Two Crowns and, yes, even in Youtube videos you can clearly see that it's not a smooth transition:

If you can't see what I'm talking about then you're just used to it and are okay with it. And that's fine, but for me personally the jagged movement of the camera hurts my eyes.
Now, for comparison, I'm going to pick some random 2D games that also scrolls the screen that has a much smoother camera transition:

I hope this comparison highlights the difference between smooth transition and non-smooth. Again, you might be okay with it, but it really strains my eyes and I've never experienced this before.
Even when the fps counter is displaying a solid 60(i have noticed it dip here and there), there is still a LOT of stutter. It wouldn't surprise me if some people simply don't see it, or it doesn't bother them, but it is blatantly obvious to me here.
I have a few other machines try this on with varying hardware and displays, so if i get around to it i'll comment again with my findings. As I'd really like to see it fixed.
It's just choppy scrolling. Not a deal breaker for most and probably not visible to those less observant of or susceptible to these things. But it really doesn't look good.
I guess I don't understand what that means, then.
