Any combo of the above causes issues.
The thing that people are complaining about is that they need to select it for every item. It is not on by default. The rebuild option was only a recent update and not there when the game was released.
It isn't about extinguisher placement and total tech minions. Saboteurs can easily destroy a room and not even give you a chance to put out the flames. The other is that you can't put out flames and repair items when you are in combat. So the items burn until you kill the enemy. They may have taken too much damage during the fight to get to and save in time.
So much this on both counts, my walls are lined with extinguishers, and they dont use them in combat, and then on top of that if we look at a lab (20+ items) or powerplant (um alot) or coms room (alot) or workshop (20+ items), and then there is like ever trap and so on, it gets quite nuts for each new room haveing to make sure they set on the rebuild per item.
Also if it was a matter of haveing enough techs is 100 not enough?
It really depends on how you build your lair. If you don't overkill on items and manage your item temp correctly, It sounds like you may be overkill on items. I have said it in other post but I get away with 25 technicians. I think that I saw the new engineering class make repairs too.
