On one of them, I sat on the screen reading the debrief for capturing, and it turns into a "You killed them" in the middle of reading it lol. Reloaded, and skipped reading, immediately completed, and got him. Silly. Also his abilities are terrible.
It's not particularly intuitive, I definitely recommend saving right before a selection is made.
As far as the discord goes (not having seen it myself) - the only henchman with 2 side missions at the end should be the ones you can choose in the beginning (Jane, Eli, Jubei, Iris).
With all other it seems to be simply the case of setting them to be captured instead of killing them. But there are quite a few reports on this bugging out.
Some times you don't get the final side quests for the henchman to get them permanently because you haven't progressed too far into main quest yet to unlock another henchman slot.
You also have to progress far enough in the game to get access to all of them. Some of the henchman it will stop before their final mission until you are far enough along with the main plot. Same as with the super agents.
