Completely agree this entirely ruined the experience for me and made me legitimately angry at the developers for robbing me of that experience with this oversight. It was from another player and I was like "huh what's this?" and ruined my achievement experience and actually finding it for myself. Massive oversight, devs. It should be JUST like the doors. Do BOTH players have the door unlocked? No? Then the door stays closed. Do BOTH players have the sword unlocked? No? Then it shouldn't appear at the start. wtf.
To be fair that IS how you get that item, so, you did get it correctly. If you picked it up on a solo run or not, the correct location to acquire it is from the vendor. Also in MP you can't go thru doors unless both players have access, so you must have had legitimate access to buy the key. Everything good there.
I think making the items not appear it's an overkill, because it robs the player who has obtained them as intended of a chance to play with this items in multiplayer just because someone else haven't found the item by that time,. I think they should make is so you can't pick up the item if you haven't got it in a singleplayer. Ideally, make this an option that the game asks you about when you enter a run in multiplayer, so you have a choice, because you might just want to play with your friend and his cool items that you don't really care about obtaining yourself.
About the "Purple Lightning" item. It's probably not from the door i thought it was, but I'm pretty sure that in singleplayer I didn't have it appear as the item that you can buy from vendor, but it did in multiplayer, on the first level.
