We considered it for a good long while, but in the end decided not to add new achievements. It would've been unfair to the players who already 100% beat the game without the DLC, and it would also make 100% perfecting the game without buying the DLC impossible if we had added new achievements.

One should be enough. There may be bugs but by design it's enough that the host player owns Melody of Mystery and then it should work for all joined players in multiplayer.

That was the Toby's Dream level, aka the dog level. It is available between the 5th and 6th levels in the main story I believe. Melody of Mystery is a full expansion campaign spanning a 6 new levels and a new story. :)
