Prologue/missing journal entries?

Any chance to get these if prologue was skipped? Is that a confirmed bud or something?
Also have one journal entry locked while its been claimed on world map and is now not highlighted, how can it be obtained?
Please advise.

I collected all the journal entries only for the journal count to be reset on three maps. Rift Town, First City and Eagle Peaks are all missing one journal each even though i have collected them before.
