Report a translation problem
After running a few tests with multiple weapons, it appears that no matter what the 'Grand Opening' mod is paired with (other mods/weapons) it will only activate once on each enemy. Successive 'first round' hits on a single enemy fail to proc the damage/explosion mechanic after working once.
As an example; shooting enemy 'A' the first time with sufficient conditions to trigger Grand Opening succeeds. Reloading and now shooting enemy 'B' also succeeds in activating the mod. After hitting enemy 'B', reloading, meeting the requirements to proc and hitting enemy 'A' again will NOT see successful activation of the explosion/damage, and has the same result for hitting enemy 'B' a second time. This does not appear to be affected by swapping/unequipping weapons or a 'phantom' cooldown, and the already successfully hit enemies seem to cancel/become immune to Grand Opening's activation until death.
That was a bit more specific, but I feel that it covers most questions that anyone else may have about this particular bug. Hope it gets addressed soon!
The explosion can only be triggered by the first applicable hit on an enemy, after which that enemy will no longer create explosions under any circumstances (though they can still be damaged by explosions off other enemies.) Alternating weapons or enemies does not help, nor does waiting for some invisible cooldown.

Just chiming in that this is an issue I'm also having and has broken a couple of my characters as it was a major source of my damage lol Haven't played in quite a long time and picked the game back up today in order to try out the changes but my builds relying on Grand Opening are all broke this way. Wasn't sure if it was an intentional change but I'm glad to see that this is being looked into and hope it can be resolved soon.
