List of Bugs
These are all the things that I believe to be bugs or glitches I have personally encountered. I will update at a later time, perhaps, if I encounter more.

All mentions of "Reset Story Point" are done on a character that has completed the main game. This was not done on a character mid-playthrough.

Main Quests;

1. Echoes;
- Uncover the Origins of the Fort -- Accessing the Journal quest step after initially getting inside the fort on "Reset Story Point" does not have the audio play as it did on a first time playthrough.
- Defending the platform/Generator -- "Reset Story Point" does not have the main character speak to Zahedi throughout this entire process. There is no dialogue as it was on a first-run playthrough. It should also be stated that throughout this entire quest there was not a single audio dialogue concerning "Monroy". The PC is silent.

2. Judgement;
- Upon first loading into the camp area before undertaking the rescue of Zahedi, the entire camp of NPCS and companions have no character model present. This bug seems to be intermittent as exiting to the lobby and then loading back in seems to fix it.

3. Sacrifice;
- The first journal you collect to find out what happened to the humans does not have any dialogue playing on "Reset Story Point" mode.
- The second set of remains is the same.
- The third set of remains is the same.
- The first set of PC dialogue from the quest occured as I reached the gate asking why Tiago/August were following.

4. Babylon;
- The entire quest the PC and NPC companions are silent except for the very first journal/remains collection. This is with "Reset Story Point" mode.

5. Humanity;
- No audio dialogue from any source until specifically entering the Caravel.

Side-Quests;

1. Free Market;
- I am on "Reset Story Point".
- The double gate that is before "lifting the crate" objective did not open. Cannot progress until I exted and re-entered the area.
- While in the same area a Red armored berserker(the ones that charge with melee weapons) voiced audio from the forest region "A trophy for Kang". Pretty sure this is a bug.

Hunter Side-Quests;
- "Hunter-Sandshift" entrance did not flash blue upon nearing it.

Lore UI glitches on separate characters;
- After initially unlocking lore entries in the codex and you create another character -- those same lore entries will appear unlocked. However, double clicking them causes the ones you haven't progressed far enough to see to lock themselves again. Such as, for instance, "Perforo". Upon creating another character I would have this unlocked until I double-clicked it and it would lock again until I visited the First City area where they are first introduced. It would then "unlock".
- Additionally, using the "Mark all as Read" on a new character after already unlocking them will mass "lock" even though it still tells me after exiting the screen at the top left that such codex entries are now being "unlocked".

Possible Glitches/Bugs;
- My character's eyes are green. Everytime a cutscene plays where the Altered eye's glow blue, after the blue fades away the eyes are no longer green, but black.
- During the quest to find Seth in the Wreckage Zone there appears to be a flash during the Outrider's visions that seems to imply there was a missing cut-scene.
- The Forest side-mission of "Rigging the Game"; upon the Outriders clasp of the man's hand the screen flashes white, implying there was a filler cut-scene here. Nothing is shown, however and thus the flash of light doesn't make any sense.
- After leaving First City on a "Reset Story Point" character that had completed the storyling the flags out in the wilderness are no longer set as if you had already used them. This leads to the impression that "Reset Story Point" was intended to be a form of NG+, yet the quest icons are greyed out in the game-world as you run around, showing that it is repeatable, but on the UI on the map menu of any particular area -- they are yellow again. However, the large world map still has them greyed out.
- Frequently it appears that if you switch to another quest after completing another one that pressing TAB will guide you to the quest you had just completed instead of your current -- this appears to be side-quest related. I have completed history quests and switched to a hunting quest and pressing TAB directs me to the Historian quest instead of the Hunting one.
- Tiago is constantly locked into a sitting position at base camp and has no head-tracking whatsoever. His dialogue sometimes has no lip-sync as well.

3 hours in the game. 2 bugs. First one was no HUD, which i fixed. Now i double click the game and the screen just goes black. Haven't been able to play it for half the day. Did everything as far as linking enix account, verifying folders, the whole list. Now what?

I've got the bug with Sacrifice quest too.I cant collect the journal at Monroy's camp.

There is a solution for it? :(

i can confirm the quest-guide bug. if you switch between quests, the guidance tends to get stuck. it's not a hughe issue looking at the map-layout, but it is still weird.
also the many white flashes during a lot of cutscenes are pretty jarring. they are often just taking a second, but it looks kinda bad.
also some side-quest npcs (the TNT-Quarry guy for example) seem to be stuck in a frozen position until you talk to them for the first time.
npcs in camp that you talk to are still visible in the backround (you can only see it in some conversations, when the camera moves) when they move during the conversation, which is just lazy....and weird seeing a Juno right infront of you while she is also sitting at her crate...

I don't know about you guys but I get this weird sound bug that makes some sort of hissing noise throughout the whole game and the only way to fix it for me is to quit back to lobby.
