Black den locked door: need sword shaped key
Summerhedge plains: some kind of key, spade shaped?
Ixeris north: keycard on valorshire temple after finishing it, go to the west side and there will be a gate that open. You will face an optional hard boss.

mistfollow forest: wing key from Ilfrig village, gain job upgrade for Clery.
In the prison: there is famous thief that will talk to Stellaire. The mission item is on Aeboros ruin. Reward is job upgrade for Stellaire.
Aeboros ruin access: from black den, you can't access it on the first time so it is easy to miss.
Aeboros ruin compass puzzle: set the compass so the NW one face the door and the rest follows it. NW compass: S. NE compass: W. E compass: N. SE compass: N.
There is a north gate on Northfalcon desert, no key
As soon as you got access to Northfalcon desert, find and complete the quest to discover the greatest mimick. Bring Medenia to the fight since the creature have a pesky buff or keep silencing it. Reward is a badge that make mimicks spawn 2x more. Arguably so important to upgrade gems.
The Kinghound on Ilfrigwood is a boss and is in the most northen hex of the wood
Ilfrigwood secret stash: when you are lost in maze, when finding tree with no leaves, there is a route that goes to a place with 3 (or was it four?) chests. No way to go to this place after clearing the maze.
Orbs: 1 on springvale acre (east side, I forgot on what I found it but it is an interesting object), 1 on ilfrigwood (inside corpse flower)
very stronk odd trunks: springvale acre, not sure the uses yet, can't be picked up.
Cogs: 1 on deathbarrow http://steamcommunity.com/sharedfiles/filedetails/?id=1334762058, 2 on first mine.
Valorshire temple: synchronizer. Seems to be job upgrade for Synch. Solaris necropolis.

Sorry, I forgot about it. Although it wasn't hidden, it is above a crate.

The solution for the compass puzzle in Solaris Necropolis:

(Location of Compass) > (Direction Facing)

NE > W
SE > N
SW > N
NW > E

Entering the room automatically pits you into a fight against some Hollows so plan accordingly. The reward is the Dial Cartridge needed for Synch's upgrade in Valorshire Temple.

I just left Necropolis, both maps (floor 1 & 2) do not show any unrevealed area, yet I am not getting 100.00% explored :(

Could it be there is some item I did not pick-up? (is the % progress bar taking also chests/items into consideration?)

Summerhedge plains: The key is leaf shaped and you get it from Rusth in the course of the main story.
And that north gate at Northfalcom desert can be opened by the lever you find in the desert. The lever opens both doors found in the desert (north and southeast). If you open one, the other closes - works both ways.

Summerhedge plains: The key is leaf shaped and you get it from Rusth in the course of the main story.

I'm at the last dungeon and I didn't receive this key. A bug perhaps?

1 cogs in Riverwell Culvert, In the room just before seeing Kishar for the first time. After you defeat the boss, the corpses dissapear and you gain access to a bag containing the cogs.

I can't manage to find the sword key to open the door in the black den. I think it's linked to Kishar's personal quest, but I can't find anyone who initiate this quest. Anyone found it?

Went back, browsed all visibly accessible areas, yet I have only 97% (even picked up the syncrhonizer and defeated droid)

I can't even spot any possible entrance to unvisited tile (when looking on the two maps above)

Did these after Valorshrine Temple . NPCs were not avaiable before I think.

Special Quest for Kishar
Go to Black Den -> S, S, NPC, do Quest -> go W, S, W, W, N -> kill Boss (you have to down all three enemies within 3 rounds. If you don't they will ressurect with around 600 HP each time (not just once like other undead)

Special Quest for Amoz
Go to Sprinvale Acre -> E, Teleport, W, S, E -> NPC, do Quest (collect 5x lumber in Springvale)

I'm still missing one lumber - are you sure all of them are in the game?
