Lower Officer Ranks?

I am curious as to what the lower officer ranks are for, lieutenants, captains and otherwise. Do they gain experience while on campaign? Do they become attached to specific units?

They are the ones that can be placed in command of new units when recruited. When the War started there were very few higher ranking officers. When the army grew from 16k to 500k it were the captains who became generals...

But how do units increase in rank? I've played a bit and didn't notice it once, my buddy with a few more hours also hasn't noticed it.

Promotions seem to be broken—I swear I’ve seen some (Col. to BG) but most people seem to stay at the lower rank.
