You play the game again and blame everyone who dies deaths on the Captain, rather than the "real" cause.

Does that include the 4 people that escaped and survived, or do I need to note these 4 as "killed by the Captain" too ?

It does nor, mark them as alive as in a regular playthrough

Just did that playthrough and didn't unlock it. Not sure what I did wrong

I ended up with the Captain having done 53 murders (since his own suicide does not count, and 3rd Mate / Captain's Steward are unknown and cannot be solved unless you solve everything else correctly first) and the Surgeon, the 2 passengers and the 4th Mate's Steward alive in Africa. What about this is incorrect for this achievement ?

Does not unlock for me either. Did the same things as previous poster.
Maybe people are trolling and there is some secret to unlocking it, like going somewhere in a memory?

What worked for me was putting 'shot by a gun by the captain' for literally everyone except the captain, for whom I put 'suicide by gun' I'm not sure if you have to assign names (correct names even) to everyone, though.

OK, putting in correct names, 1 suicide, 2 unknowns, the rest are shot by captain worked.

If you report that some people are alive, you can't also blame their death on the captain ;) The final report should say that the captain killed everyone. Except the 3rd mate and captain's steward, because they are locked.

"eaten by the captain" wrong names to most people to every crewmember, even the ones alive, the Captain hung himself. that worked for me.
