Can't find the [spoilers] head

Can't find the robot head. Even looked it up on the Wiki - it says there's some kind of hole on level 5 (Security) which leads to level 3 which is where you find the head. Well I've cleared out* all these levels , searched every nook and cranny to the best of my ability yet still can't find it.
*Well actually in Security Quarters there's a "remote controlled door" I can't open (some Mind worms are highlighted behind it), I'm guessing this is related to Hargrave's quest? I've never met him, never got his quest (only saw him when I was rooting around on that level not knowing how to get to him, and when I was finally able to reach his area, he was gone). Please don't tell me the head is behind that door and I've somehow soft-locked myself out of it.

Keep poking around you are definitely missing out on some areas... its like a blood maze out there with several secluded vertical shafts like ares that are only initially accessible trough a single obscure access point.

in particular: Robot Head can be extracted from a broken robot in the third level of Mission Control with Computers:[3] (can be only accessed through a hole in Level 5 - Security Quarters) <-- its during a quest from Hargrave (you access it either by going past the mind worms or from a hidden vent(perception check must succeed) located in a room adjacent to a bath room...

You could have met Hargrave by interacting with the crate barricade right next to the forcefield room. I strongly recommend enabling permanent outlines in the menu (or holding TAB) to see what objects can be interacted with.

If you're in Chapter 3, you can safely enter the mindworm door, or there's another entrance below and to the right accessible with Perception 8 (you can use your faction companion for this if no one in your party has PER 8).

Beyond these doors there's a hole in the ground, and after judiciously exploring around you can find the head.

Thank you guys for replying. Still haven't got it unfortunately. i linked a video below exploring the areas so people could pinpoint the spot I missed.

I am in Chapter 3. Unfortunately it seems I have missed Hargrave entirely. He's not in the "forcefield room" in Operations, interacting with the barricade didn't do anything. I wonder if doing his quest is necessary to obtain the head, i.e. if he unlocks that "remote controlled door" (1:05) in Security Quarters and behind it is the hole that the wiki mentioned. Which is going to be extremely painful because it means I am now hard-locked out of some potentially exciting content.

I have only encountered perception checks twice in the whole dungeon, both on the Admin deck: once to find the hidden vent in the room with red barrels (0:10 in the video), another time during the quest to find Jimmy (near the entrance to the Admin deck). All the time at least one of my companions had PE > 8, currently it's at 11 (I confirmed in another thread that PE checks are made against the highest PE stat in the party).

I don't know what else to do. I made a video of all the relevant decks to see if anybody can pinpoint the location I am looking for:

Go down the big highlighted hole on the right of the screen at 0:22. You even got the PER check, it's that tiny hatch you can see on the right at 0:20.

Oh my god thank you Skaudus and Grraf. I'm actually dumb. I DID miss it - rather, I decided to put it off til later and then forgot about the fact that I haven't actually gone down.
