Be aware that before I started to play this game I had NO FREAKING IDEA of what a tire size was
So while I indeed gently poke you, remember I had to go myself through the process I'll try to guide you into.

YOU complain the tire is thin and you choose yourself a very thin size...
So maybe understanding what you are doing first ? I would truely consider some reading there

Now ofc, you may read only the Tire Width - Aspect Ratio - Wheel Diameter parts, As only those are in game implemented.
Now you start with a car, and you want to change rims (for bigger) and a thinner sidewall of the tire on the rim... BUT you have a overall diameter to work with, as in this game you can't make changes to the body parts... so a too big wheel may look ugly in comparison of body placement.
That's where a calculator site becomes handy
Like maybe this one (complex looking at first, but once you do understand things... great tool)

So let's say initial tire size on your car was a 225 / 75 / 14''
And you want a 245 on a 18'', you have to know what is the aspect ratio to keep around same diameter...
Closest one would be a 245 / 50 / 18''
Now IF your tire is too much on the outside of the body of the car, and you don't like the look, it's where the offset comes into play

And if I'm not misstaken (but there I could be wrong, so please correct me if I am) in this game we have only the positive offset, which makes a too wide tire beeing "pushed" more toward the interior of the car.

I NEVER sell my tires of a car while restoring before I have indeed made my decision and bought new tires. that way you can always quickly check what was the original size.
If restoring a car for selling, I ALWAYS repair rims BEFORE buying tires... if I break 2 rims out of 4 it's usually when I go check into my warehouse and see if I have an interseting different set of rims (that could btw differ in size, so if I buy tires first...) that I may have bought from the junkyard and repaired...

But I think I got why you are a bit lost...
When setting "unrealistic" you get the game placing you on the tire shop at the very first width of 135. You can change it with the arrows...
Same goes with any changes you want to make.
