To get the crankshaft out, you have to remove the rod caps, pistons, etc. like other people said. You also have to remove the transmission. You'll need to raise and lower the car several times if you're doing it with the engine in the car. You can only access the pistons from the top, and the rod caps and crankshaft from the bottom.
The hoist though remains a mystery, do I have to spend a talent point on it directly - I still can't find any mention of it on the talent tree? If so, I think I've got more interesting things to spend points on.
IIRC it unlocked with the second garage upgrade (two lifts).
Because the crankshaft is supporting the conrods and pistons in place against gravity so if you tried removing the crankshaft first then all pistons and conrods would lose support and slide down the cylinder tube falling to the ground.
That and you can't budge the crankshaft while the conrods are sitting in their "slots" as they will block this movement (or everything gets scratched and dented), so you'd have to push the conrods up and rotate the crankshaft using all two of your hands... it won't be pretty and the chance of scratching and damaging something is high, so it's cleaner to remove the pistons first.
