1. I only see crosses in the lower-left of the screen. Am I confused about where I'm supposed to be getting crosses? Is there some difference between lower-left and lower-right crosses? What is it?
2. You say "its only supposed to show 3 crosses." Ok, Good, then all I'm supposed to do is get 3 crosses and then get a few more answers correct. (I had misunderstood the ambiguous, one sentence summary in nz_2683's guide.) But as I said inOP, I've done that! I've gotten 3 crosses many many times, and the achievement isn't unlocking. So, I ask again: what am I missing? Don't you only get crosses if you're getting answers correct in .6 seconds? If I'm getting 3 crosses repeatedly, is there any reason to think I'm consistently effing it up and not getting the last one?
3. But you just said there are only 3 crosses that could ever show up, and I said in OP I've done that. So what would an "additional cross" even be? If (as I said in OP) I'm seeing 3 crosses show up and then getting more answers correct at the same rate, then (as I asked in OP) what is it that I'm missing?
However all the answers to those questions go, these others remain
4. Why am I able to toggle "sherlock mode" on and off in the menu if I don't have the achievement? I thought the idea was you unlock the mode when you get the achievement.
5. Since I can toggle the mode on and off, I can report that it doesn't seem to do anything. So again, what am I missing, or is something broken?

So, to summarize, here's what my confusion was:
It's possible to get a combo and the 3 + symbols without answering at an interval of .6 seconds. So simply getting 3+s is not an indicator that you are answering quickly enough to unlock Sherlock Mode by answering an additional few questions correctly at the same interval you've been using to get the combo.
(Still not clear why I have the ability to toggle sherlock mode in the menu if I haven't gotten the acehievement -- that seems a bit buggy, no?)
I've just done this, twice. The first time, I had Sherlock Mode toggled off (no "X" ) in the menu, and the achievement did not pop.
The second time, I went and toggled Sherlock Mode on in the menu (put an "X" next to it). This time the achievement popped.
Maybe this isn't a "bug" per se, but it's hella counterintuitive: I have to toggle Sherlock Mode ON and then do all this in order to "Achieve Sherlock Mode" ?
Yes, you have to toggle Sherlock Mode on in the options to achieve Sherlock Mode. It isn't a separate gamemode that enforces other rules, it's a gameplay mechanic to reward players.
If you have it toggled off, you won't be able to get Sherlock Mode. If you have it on, you will be able to get Sherlock Mode.
And the 3 crosses in the lower right corner are your combo. For those you do not have to get guesses right with less than 0.6 seconds in between, the timing is much more lenient. Only when you have the full combo, you have to get 4 more guesses at <0.6 seconds. The initial combo only increases the amount of frequency you get from your guesses.
