I don't know enough medical science to comment on how realistic it is, but in universe she has psychological damage that expresses itself as blindness.
They discuss this a number of times, actually, as well as the idea that it could wear off over time.

Different People means different preferences! For me i don't know how people like From Software Games or The Last of Us, but there are many so yeah, deal with it.
It's got great overall charm. The setting is fun, it doesn't take itself too seriously, but at the same time, it's filled with drama and cool story ideas.
There's copious content. Everything from loads of side missions and minigames, to an impressive list of heat actions. The overworld is nice and compact, with tons to do and almost no dead space.
Good idle mechanics. Both characters have their own idle mechanics--those being the real estate and Tiger&Dragon--which are unobtrusive, optional and mostly fit with the high granularity of other side objectives. One of them is even variable-time.
Includes Sega classics also. I had never played Outrun or Super Hang-On before. Both great. Not to mention Space Harrier. (Let's not talk about Fantasy Zone though.)
My only gripe is that the combat is a little underwhelming. But even then, it's more than serviceable. Just, nothing special.
I actually have a rescue pig that has the exact same condition Makoto has.
It's psychological blindness induced by trauma - Absolutely nothing is physically wrong with their eyes, they just "don't see" anymore. In humans, it's relatively easy to treat due to therapy, though obviously depends largely on the extent of the trauma. In animals... it's nearly impossible to treat and you just kind of have to hope they'll eventually get over it, or just help them live the best life they can.
For my pig, the cause was witnessing her previous owners slaughter the rest of her family in front of her. (...and yeah, for anyone that doesn't think pigs are intelligent or have feelings... the fact they can develop legitimate psychological issues from trauma is... quite the sign that they do feel things just like we do.)
For Makoto, it was assault, abuse, torture, etc. combined with a complete loss of hope.
So yeah. It IS a legitimate thing. It's just fairly rare to see in humans long-term.
And wow that sucks for your pig, like damn.
