Can anyone explain to me how do these games qualify as age restriction +18?

Since 0 is often regarded as the best game in the Kiryu saga I decided to ask here.

So yes can anyone explain to me the +18 rating these games have because I simply do not understand.
I mean hardly anyone dies in these games and when they do it's usually off-screen.
Or when the games present something "disturbing" it's usually not shown directly. I mean like in Yakuza 3 Mine brings a severed head in a briefcase but it's never shown directly, pfft seriously Oblivion has that and it's rated +16.
So what is it then? The alcohol consumption, or smoking? The Gothic series games have that and they are +16 too.
These game's don't even have any nudity despite having strip clubs in some entries.

So what is the reason then? The few swear words they use occasionally?
Seriously can anyone tell me the reason for the +18 rating, because I don't get it and I really wish to know.

Im disappointed it didnt get a follow up in kiwami 1 or 2 where she would punish him for not visiting him again like she ordered. :V
