[Spoilers] Man in Black Achievements

I got an achievement for saving the Man in Black and one for letting him die. How's that even possible? Was there an option to save him? Because for me he just fell down into the fire.

Oh, I didn't know that. Does it change the story or anything like that?
I got both achievements even though he died, so..
