It all goes by console commands.
At least, if someone has used cheat console on a character, maybe put something to show that on their profile icon or something. So that people know they've used cheat commands. I'd say for the most part people want to play online and legitimately beat the game.
1 person out of hundreds, no need to avoid multiplayer, I just kick them if it's from using cheats.
and I donno how you'd have a level 55 character with 0 xp on other characters providing buffs for the difficulty so id call sus
