I'm working on getting 100% complete and I can only find 11 of the 12 codes for the dispensing machine. The ones that I have found are

The locations are marked on the map on this wiki page. There is a secret 13th code too ;D

do there a paper we can loot for secret code in this floor or anywhere ?

no - there is no paper the player can pick up for for "secret code" #13.
I did add the secret code on the wiki page quest description though.

i'm working on this too and can't find Magic Incantation, can anyone help me? that map isn't really useful to me.

Why isn`t that map useful to you? Do you have trouble seeing green color?
