Crafted a greeen 1-h sword.

Put it in the forge, highlighted base damage and clicked below on the "upgrade" button.

Now, there is an option of item forge in inventory. That one is to "fill" those forge slots i`ve mentioned. By doing this, you increase chosen stats of items. Select this option, move some item to the right pane, select some stat from the list below and upgrade it.
Hope that clarifies both forge mechanics for you.
Yes you can upgrade them using the Item Forge button. You have to be in town to do it. Then when you click it the right side becomes the forge interface.
Drag the item you want to upgrade into the slot and the properties you can upgrade will list below. Click on the property you want to upgrade in the list, then click the Upgrade button that shows what the cost will be.
