Mission 3 safe codes not fibonnaci.
I've read multiple guides stating that the 5 safes inside the main vault use fibonnaci as their codes (011, 235 e.t.c), yet they don't work for me. I've brute-forced my way through all 5 of the safes, and the codes have been 719, 129, 269, 696 and 818. I feel like I've missed somthing important here.

Alright sir, do the numbers 719, 129, 269, 696 and 818 appear in the sequence?
Because from this guide (http://steamcommunity.com/sharedfiles/filedetails/?id=1135510717), which I assume is where you got it from, the number 7 never even appears within the first 6 3-digit codes.

That's the set of numbers I got and have been the same for all 4 of my playthroughs. I don't know what's different about your game. Strange.

I feel like my game's bugged or something, it wouldn't surprise me with such a new release.

Maybe, it may also be some kind of fail-safe to keep you from running right to the vault? Like "you must find this note or the combo will be randomized" kind of thing. I seem to remember somewhere a book in the bank talking about someone being close to cracking the vault.
Aside from something like that, that's an annoying bug, but it's not game breaking, you only lose out on a bone charm and some coin, so that's something.

I'm sure I scourered the bank from top to bottom for every scrap of information, as I tend to do when trying to go unnoticed on my first run. I'm sure I would have ran into it, although I just tend to pick it up and go.

Yeah a note earlier in the mish mentions something about the numbers following some type of sequence ... a series: it's fibonacci. At first it didn't work for me but then I realised that you have to account for safe number 4 (which has no dials), not simply skip it. Every time you enter the correct set of 3 numbers on a safe, you will hear a faint kinda-click sound. 011, 235, 813 (skip 213 for safe no. 4), 455, 891.
At least this is what worked for me.

For the third time, the sequence did not work. I already listed what numbers I found that worked. When I put in those numbers the safes still made clicking sounds as if they were correct, but they never opened.

They are, the safe that holds the sword counts as part of the sequence as well. I was stuck for awhile until I realized that.

No worries my dude, I was staring at those safes really and getting pissed off for a good 15 mins before I tried that lol.
