You didn't play the canon version of Dishonored 2 then if you don't know why she still has her arm and Eye.
Canon events of Dishonored 2 are low-chaos, and Emily *stops Stilton from attending the seance*.
Thus, back those 3 years before D2 took place - in the era that you flip back and forth from in the Manor - Stilton wasn't vanished, didn't go insane, and didn't die. When Meagan goes to visit him, he's there, she's welcome, the place isn't crawling with guards and they don't harm her.
However: if you carefully listen to the outsider's comments in the Hollows scattered around the maps in DotO, and piece together things that Billie says to herself, BOTH of those timelines are *correct*.
In one, she loses her limb and eye, and Stilton is mad. In another, she does not. However those events *still happened*. The Outsider is working with that 'damaged' time line.
It's *amazing* what effort they went through to put that level together.
Spoilers, because you totally should see this for yourself :D
The manor itself has 4 variations: the rundown 'present day' version you enter the mission with, the 'party in the past' version, the 'you killed him' version which is 'it got sold to a guy at the party but he's selling it again and it's empty because creepy stuff still happens there', and a 'you saved him / canon low chaos version' where it's present day but FIXED and beautiful, with people in it, a new addition, and remodeling in progress.
