From the Dishonored Wiki: (And my own playthrough.)
Should the course of history be changed by preventing Stilton from going mad and disappearing from public, Meagan will have regained her arm and eye since the fight at his manor would have never occurred. She brings the protagonist back to Dunwall after the Duke's elimination and recovery of Delilah's soul to reclaim the throne and to end Delilah's reign of terror.
So, since she is missing her eye and arm they have apparently chosen not to go with the absolute best ending this time. They have gone low chaos, but without the repairing of time.
Yes that's what happened in my low chaos play-through, I left Stilton alone and yeah that's what happened
So yeah what you guys said that corvo or emily must of killed stiltion, or left him be and let him witness the horror.
