Fallout VR is pretty epic so far but my goodness they need a tutorial somewhere! Or at least a control map chart!
I have put the game on hold for now until they fix a few of the bugs as its leading to a poor user experience at the moment.
I have to wonder, though, if this is everyone's experience, or just an unlucky few.

It does try to select body parts that you are pointing at, but its REALLY bad at it.
I could never get it to target a head even when I had the muzzle of my gun point blank in their face.
It would also constantly flip between several body parts and could never really figure out what I was actually aiming at...
The solution is of course to completely disable the retarded selection system.
In your documents > My Games > Fallout4VR folder, edit the Fallout4VrCustom.ini file.
Insert (or modify if you already have it)
fVATSTargetUpdateSec=999
into the [VATS] section.
This will completely disable the target selection and turn VATS into pure bullet time allowing you to freely aim as you normally do without the broken selection system.
Its not just you, i find it easier to aim outside of vats than to actually use it for what its for. It jumps targets that are not even close to each other, not to bad if you only have one target but it still jumps from limb to limb so fast it makes it pretty useless. Its just something else that broke when they 'transfered' the game to VR and not 'converted' to VR.
I normally get close enough, fire up vats to aim, then disable it, fire and re-enable it to get those head shots.
It's different for each controller - you'll have to look at the VR controls section of the help in-game to see for sure for your VR set. For what it's worth, I use the old Oculus Rift-S and there the VATS button is "B", which is the secondary thumb button on the right hand - push "B" to enable VATS mode, then aim at the various body parts and use the trigger to execute each shot.
