How do I get the costume? I have two civvies standing there and a guard neither of which move an inch

use the coin to distract the civ`s, the guard view shouldnt be a problem.

Or use showdown mode to knock em out after clearing the guards

use sleeping potion of Doc for Civ and shoot the guards

Cooper can just knife throw and knock the other out instantly after mccoy shoots the guard.

First of all it depends if you play on Hard or below.

If you play on normal just synchronize actions. Throw knife on guard and in same time doc and cate stun/kill civs.

In Hard you have one additional guard. So you need to synchronize your characters to eliminate all of them in same time.

Kill the guards below by luring them using footsteps/coin into the bushes.

Then use Hector's footsteps to lure the long coat down there.

Then on the two remaining ponchos near the dress, coin toss one of them from the bushes, kill them both synchronised.

Alternately, just use Cooper's coin to distract the civvies, and Kate's perfume to blind the guard. It's definitely possible to do those two things and then have Kate grab the dress without getting spotted, even on hard.
