Report a translation problem
I tried on both normal and difficult. On both I am exactly 11 enemies short. No civies dies. Entire map cleared.
I also noticed that reinforcement don't count. I mean they do, if you trigger it, but it just raises a total amount of enemies on the map which you have to kill (again according to the final summary). On my second run I played on hard, didn't trip any alarm but I was short by 11, just as on my first playthrough when I cleared every guardhouse.
My current theory is that you also have to kill specific goons before Cooper meets Kate. While killing some enemies will remove them from later stage, I am pretty sure some of them are this early game specific and get removed later on.
For example, when you go intended way there is this guy under the boulder who guards the enterance from the side path. I usually kill him if I take that path.
However, I didn't notice his extra presence when I did the achievement to go through the main balcony. The next time, I will go normal, and kill every bloody guy I can before triggering the meeting. WIll see if it will help.
So Guards from guard houses only count if you trigger them. Only enemies on the map need to be killed. If you didn't raise an alarm you don't need to worry about guards who didn't come out.
So the trick is: Some guards from the "Just-Cooper" sequence don't appear after you are joined with Kate. Some of them will, but it seems that about 11 of them won't. Those were guards I was missing.
So to get the badge you need to clear the map twice - once with just Cooper, and then with Kate.
If you can't kill Coats with just Cooper, don't worry, they don't respawn - so killing them at the end should suffice. Though you can shoot, circle around their vision cone and then stab them with knife them while they are still stunned just fine. Easier with gatling gun though

68/69... one is missing but HOW?
as solo cooper I killed everyone exept 2 Coats, with Kate killed everyone... bug? :(
Are you sure you didn't knock and tie up someone why accident? Did you make sure to use minimap to locate every enemy?
My initial goal was to murder everyone with Kate, then every one with Cooper and everyone with both. I quickly realised it would be impossible to do with just Kate, so I left it as it is hoping Cooper will be enough. However, before that I did perform one kill, and that guy did respawn for the Cooper bit. It is possible that it is the guy you were missing (if that is the case, I was lucky as hell, and that's really really really stupid design).
So do you remember the guy who hangs out under that hanging rock - he guards the enterace to the "photo shoot" area if you go as intended with Cooper. You are likely to kill him and hide him in the bushes as if he goes to watch the enterance from the side path he is not watched by anyone.
After starting level with Kate, I kicked him, and threw him over the railing. Later, when Cooper appeared the guy was there again. Perhaps he is the single kill you need?
This is only explanation I can think of. As solo Cooper you must few times throw a knife and then qucikly punch/stab another enemy. But stunned enemies have BIG icon about being unconscious...
Or maybe that one missing enemy just ran inside some building as part of some buged script?
Now I'm not even sure if my halfway save with Cooper only is flawless or not... I'm tired of this mission - vanishing of some enemies and resurrecting other enemies after cutscene is VERY BAD IDEA.
Maybe? As solo Kate I didint kill anybody. Just run, kick, kick, kick, kick and to the office.
but now I'm too tired of t his mission to play it whole AGAIN, even on easy.
If you accidently tied someone up it's easy t forget to stab them later.
