Actually, Rampage is in a place you don't have access to, that's why you can not see him completely, but thanks to the kindness (or cruelty) of a boss, you'll be allowed to see him again... but not completely.

I personally find it interesting that despite Fury's assertion that "The Horseman is not defined by the horse", the enemies of the Horsemen have been making a remarkable effort to rid the Horsemen of their steeds. Ruin was captured and nearly broken by a Demonic Horsemaster after all, to the extent he was fully trying to kill War for a while, and Rampage was actually assassinated here. Presumably the only reason Despair didn't suffer the same fate is because Death kept on summoning him across multiple different worlds (or he was hiding out in the relatively neutral Land of the Dead, which wouldn't surprise me).
