Can't access Pride, possible glitch?

I was fighting the second Wrath and the moment I killed him my character Fury suddenly pops up outside of the area you are fighting him. He is no longer accessible and I didn't collect his soul and there was no cut scene. He died, I did try to go back to see if he came back but the area was no longer accessable. I now have a Green skull on my map. I don't know who that belongs to? I attempted to go through the portal to Pride but she says I am not ready.

Sounds like a game breaking glitch or am I missing something?

That's pretty much what happens when you kill him, after the cutscene showing how you finish him off you find yourself down there with the lift gone. You should have received a Luminous Visage for the victory if it counted.
As for the boss order, can't say off the top of my head if you could have skipped any by that point in the game, I only finished it once and waiting for the DLC now.
You should check against the list of bosses whether you're missing one, otherwise - it's a bug.

I didn't receive any cut scene or Luminous Visage. After the final hit that would have killed him I just appeared somewhere else on the map, somewhere not far from where I first entered his area. Now I can't return to him.

Does anyone know what Sin is the green skull? That is where its telling me to go. I thought it was Envy but it doesn't look like the level with Envy is accessible anymore. Too far back in the beginning of the game. I don't think I can go back there either.

Alas, that definitely sounds like a glitch, there should have been a cutscene and you should have appeared right next to the spot where the lift up to the arena was with a Luminous Visage in hand.

Green skull is Sloth I think. Back in the train station area.

Yup. You get the Storm Hollow after beating either Sloth or Avarice, who are the next two available Sins following your meeting with the Lord of Hollows. After that, you can fight the other of the pair whenever you like.
